How to request a name change

Before you contact us, please have a clear digital copy (a high-quality photograph or a scan) of your official name change document ready.
Acceptable documents include:
A marriage certificate.
An enrolled deed poll.
A decree absolute, which must be provided alongside your birth certificate.
Documents must clearly show your full legal name and be issued by an authorised body.
